Please read and complete all required forms prior to your first appointment. These forms will be made available to you through a HIPAA compliant portal once you have scheduled your first appointment and created a secure login via the Therapy Appointment platform. Your therapist will explain the necessary steps during the 15 minute introductory call once you determine that the therapist is a good fit for you.

Our therapists are in network with Blue Cross Blue Shield, Medicare, and United Healthcare. We are considered out of network with all other insurance companies, but are happy to provide you with a form to submit to your insurance so that you can be reimbursed if your policy allows for out of network benefits.
